Custom Name Activity Worksheet – Printable PDF

Palm & Play Custom Name Worksheets are digital products only. No physical worksheets, supplies, or materials will be shipped.

After your purchase, please download the Palm & Play Custom Worksheet Roster Form from the confirmation link, complete all required information, and email the finished form to info@palmandplay.co.

Once your roster and customization details are received, your worksheets will be created and a digital proof will be sent by email for review. You may request revisions during the proofing stage before final approval.

After your proof is approved, the completed printable PDF files will be delivered to the email address provided with your order.

Please double-check all names, ages, grades, spelling, and customization requests before submitting your roster. Larger classroom orders or extensive custom designs may require additional processing time.

Nothing will be mailed — all final products are delivered electronically by email.

Palm & Play custom worksheets are personalized digital products created specifically for each order. Because of the customized nature of these products, all sales are final once design work has begun and a proof has been prepared.

A digital proof will be emailed to the customer before final delivery. Customers may request reasonable revisions during the proofing stage. Please carefully review all names, spelling, grade levels, design choices and other information before approving your proof.

Palm & Play is not responsible for errors resulting from information submitted incorrectly on the customer’s roster or order form. If Palm & Play makes an error that differs from the information provided by the customer, we will correct it at no additional charge.

Once a proof is approved and the final PDF has been delivered, the order is considered complete and is not eligible for return, exchange or refund.

If you need to cancel an order before customization work has started, please contact info@palmandplay.co as soon as possible. Refund eligibility will depend on the status of the order.

No physical product will be mailed. All final files are delivered digitally by email.

I’d also add a separate “Proofs & Revisions” info section right after this one. That will make your policy feel much friendlier because customers immediately see that “final sale” does not mean “you’re stuck with the first draft.”

Each worksheet may be customized with the child’s name, selected worksheet style, font preference, age/grade information, theme, learning objectives, and other requested details when applicable.
